Hello everyone. This yellow form was sent out this week with students. This form is a consent form for counseling services. It is very important for me to receive it as soon as possible. Please choose I do or I do not and sign please. The form explains the services that school counseling provides to students. This is from the counseling department/district. Thank you. :)

Hola a todos. Este formulario amarillo se envió esta semana a los estudiantes. Es un formulario de consentimiento para servicios de consejería. Es muy importante que lo reciba lo antes posible. Por favor, marque "Yo consiento" o "Yo no consiento " y firme. El formulario explica los servicios que la consejería escolar ofrece a los estudiantes. Esto es del districto y departemento de Consejeras. Gracias. :)

September is Childhood Cancer Awareness Month — a time to honor the children and families affected by childhood cancers and to emphasize the importance of supporting research on these devastating conditions. Together, we can spread awareness and hope.

Texas Senate Bill 12 requires all students to have a health consent form on file to receive ANY health-related services, including first aid and illness care while at school.

If you select "NO" on the consent form, nurses/staff will not be able to treat your child if they are injured or sick while at school and a parent or guardian will have to pick up the child. *This does not apply to life-threatening or emergency situations *

Consent forms will be sent home with all students, forms will also be available at all schools. You may also fill out the Google form below to provide consent:

El Proyecto de Ley del Senado de Texas 12 requiere que todos los estudiantes tengan un formulario de consentimiento de salud archivado para recibir CUALQUIER servicio relacionado con la salud, incluidos primeros auxilios y atención por enfermedades mientras están en la escuela.

Si selecciona "NO" en el formulario de consentimiento, las enfermeras/personal no podrán tratar a su hijo/a si se lesiona o enferma mientras está en la escuela y un padre o tutor tendrá que recoger al niño/a. Esto no se aplica a situaciones de emergencia o que amenacen la vida.
